Two boxes of metoprolol 25mg and 50mg were shelved next to each other. A 50mg box was picked for a 25mg script and caught at the final check before handout.

Done straight away: Correct strength dispensed. The two strengths have been separated on the shelf with a divider and a shelf label added.
